Treatment Advocacy PHP's HIV positive participants are able to access the following continuum of care: Coordination of Services: The PHP Case Manager and the participant determine a treatment plan that is constructed with attention first to the participant's priority of needs. Housing may be at the top of the participant's list or even just receiving a clean pair of socks. The Case Manager becomes the focal point for ensuring that the participant receives all the identified andreferred services. From previous experience, PHP staff usually provides one/one care coordination over a minimum period of six months with bi-weekly appointments.
