Prevention Case Management
Development
of a Prevention Plan:
Together, the participant and Case Manager develop a written individualized
Prevention Plan which highlights the services that are important to the
participant and the behavior changes that have been mutually agreed upon
during the counseling session. The Prevention Plan addresses the identification,
coordination and delivery of one/one HIV prevention services, as well
as the assistance needed to obtain STD, TB and Hepatitis diagnosis and
treatment and any other primary health care services related to HIV prevention.
