Prevention Case Management
Prevention Case Management includes highly individualized counseling, crisis management, and facilitating the access to appropriate medical diagnosis and adequate primary medical care treatment, as well as to other services both onsite and at other organizations within PHP's extensivereferral network. Operating as a team, the PHP Prevention Case Managers and a Crisis Intervention Specialist encourage participant's long-term engagement in care. What PHP participants can expect when working with a Prevention Case Manager includes the following activities:
